Rehab Scholar Intern

Working under the direct supervision of VNSNY Registered Nurse(s) or Specialist(s), theRehab Intern will be responsible for participating in one or more of the following areas within a specific program: Service Delivery, Health Education, Operations and other Learning Experiences. These experiences will focus around the transition of care as a patient prepares for discharge from the hospital to the post acute care – home care program. The Intern may assist in: 1) the identification, assessment and delivery of patient care 2) the creation of effective teaching materials and beneficial health promotion activities for patients consistent with Agency philosophy, policy, objectives. 3) the review of documentation to ensure accuracy and compliance with regulation 4) the coordination of day-to-day activities 5) entering, collecting, researching, compiling and/or verifying data and developing, running and/or monitoring related reports 6) identifying problems and resolving/escalating issues as necessary.

Qualifications: Required interest in Home Health Care and Community Health as a Career.

Education: Current enrollment and one-year completion in an accredited program inPhysical or Occupational Therapyrequired. Cumulative GPA 3.3 or higher required.

Skills: Proficiency with personal computer required. Intermediate level Word and Excel proficiency required. Effective oral/written communication and presentation skills required.

Nurse Scholar Intern

Working under the direct supervision of VNSNY Registered Nurse(s) or Specialist(s), the Nurse Scholar Intern will be responsible for participating in one or more of the following areas within a specific program: Service Delivery, Health Education, Operations and other Learning Experiences. Qualifications: Required interest in Home Health Care and Community Health Nursing as a Career.

Education: Current enrollment in accredited program for Bachelors of Science in Nursing required. Cumulative GPA 3.3 or higher required. Successful completion of Medical-Surgical/Adult Care junior level nursing courses and clinical rotations.

Skills: Proficiency with personal computer required. Intermediate level Word and Excel proficiency required. Effective oral/written communication and presentation skills required.
